Please print out and fill in the application on the Membership Brouchure.

Ensure that all boxes are ticked as to your approval and consent of the

Association's Code of Ethics And Practitioner Guidelines.
The Approved Standards and Personal Attainment Form,
also needs to be filled in to your level of Attainment.

* Post the filled in Application form from the membership brouchure
together with the filled in Personal Attainment Form.

* Please ensure that you enclose photocopies of Reiki level Certificates
and Linage if possible.

* Pay by cheque or contact us for other arrangements.

Please note: that upon receipt and approval of your application for membership,

Membership Certificate, membership number and receipt of payment
will be forwarded to you.
